My Health Pays is available to Ambetter Health members and is a rewards program that pays you for the healthy decisions you’re already making each and every day. 

Completed your annual wellness screening? You get points for that. Want to learn new ways to be healthy? You can get rewards for that, too.
After you've enrolled in an Ambetter plan, the only thing you need to do is log in to your account and activate My Health Pays. That’s it!

Then, you can start earning rewards every time you complete an activity.

After you activate your account and start getting points, you can convert them into money to help you cover health-related costs and monthly bills such as:

If you’re 18+ and the primary subscriber of your medical plan, you can sign up for Take Control Rewards today. Just follow these three easy steps and you’re in:

1. Visit myCigna.com.
2. Log in or create an account.
3. Take the short Health Assessment via the “Wellness” tab. Once it’s complete, click the “Start Earning Rewards” button to be taken to the Take Control Rewards website.

1. The Cigna Take Control Rewards Program is available in all states to all primary subscribers who are active Cigna Healthcare Individual and Family Plan policyholders and who are 18 years of age or older. All

rewards may be considered taxable income. Contact your personal tax advisor for details. Program participation along with reward redemption is dependent on qualifying premiums being current and fully paid.

3. The Reloadable Reward Card is just one example of what may be redeemed with earned points in the Cigna Take Control Rewards program marketplace.

4. The Health Assessment must be completed on myCigna, and all four behaviors associated with the Annual Wellness/Preventive Care tile on the Take Control Rewards site must be completed. Points for the

Wellness Exam and A1C Test are applied 10–15 business days after the health care provider submits the claim and that claim is received by Cigna Healthcare. There can be significant delays with getting your

points added to your balance as it depends on when your health care provider submitted the claim to Cigna Healthcare.

Molina Healthcare: My Health Perks

Benefit: up to $200* Gift Card!

My Health Perks gives you access to wellness programs, healthy information, and a $200 reward for all states except Washington. The reward amount in Washington is $100.

My Health Perks Eligibility:

The program is voluntary and available to all Subscribers at no cost. The program is additionally available to Dependents 18 years and older at no cost.

Earn a $200 Gift Card:

    • Complete the two steps below to help Molina better understand your health needs and earn a $200 gift card.
        1. Tell Molina about your health – Log into your My Molina Portal and visit the My Wellness page to complete the My Health Perks Molina Wellness Assessment
        2. Complete your yearly physical – Visit your Primary Care Provider (PCP) for your annual Wellness Examination.
    • Claim your reward: We'll notify you via email once you’ve completed your online wellness assessment and your annual wellness exam claims have been received. This may take several weeks.
    • You can track the status of your PCP claim anytime on MyMolina.com. Visit the My Health records page and click on claims history. 
    • We'll email you when it’s time to select your digital or physical gift card.

Benefit: $100 Prepaid Visa Card

Members age 18 and over can earn a $100 reward on a prepaid Visa card upon completion of five easy account-related activities through UHC's "Health Perks" program. Earning your $100 prepaid visa card is simple - the required tasks include:

  1. Visiting your Primary Care Provider (PCP)
  2. Registering for an online account at myuhc.com/exchange
  3. Opting in for all types of text alerts
  4. Signing up for all paperless communications (must opt-in to all email types)
  5. Setting up Autopay to pay your monthly premium (not necessary if you have a $0 monthly premium)

The reward can be earned by each adult enrolled in the plan. Members can use their 2024 reward anywhere Visa is accepted!

*If a member has questions about receiving their gift card, they can call RMHP Quality Department at 1-855-830-1565 or 970-263-5552. If no one answers at the time of their call, they can leave a VM and someone from the RMHP Incentive Team will return their call. 

Benefit: up to $100* to redeem at CVS!

Aetna is excited to be the first carrier to offer an over-the-counter allowance to their members, better known as OTC. By creating a connected experience for Aetna CVS Health members in store , OTC is a value-added benefit focused on health and wellness.

Members will receive $25 every three months while they’re covered to redeem at CVS. That's up to $100 a year!

All new members, beginning 01/01/2024, qualify for this benefit. The enrollment is automatic, there's nothing you need to do to sign up. The funds are automatically added to your ID card once per quarter.

This allowance can be used in-store, online, and over the phone. Members will have access to an in-store catalog with more than 900 items to choose from!

What is the Over-the-Counter (OTC) benefit? 

The benefit lets you get over-the-counter items by going to any allowed stores. You can also order by phone at 1-888-628-2770 (TTY: 711) Monday to Friday, from 9 AM to 8 PM local time or online at https://www.cvs.com/otchs/myotc. You order from a list of approved items, and it will be sent to your address.

How much is my OTC benefit? 

The amount depends on your benefit plan. Check your plan documents or call OTC Health Solutions at 1-888-628-2770 (TTY: 711).

How often can I use my OTC benefit? 

Your OTC benefit can be used once throughout the quarter if you order by phone or online. However, you can use your benefit unlimited times at any OTCHS-enabled CVS Pharmacy during the quarter. 

Quarterly benefit periods are distributed as follows:

    • Quarter 1 (January, February and March) 
    • Quarter 2 (April, May and June) 
    • Quarter 3 (July, August and September) 
    • Quarter 4 (October, November and December)

Note: Online or telephonic orders are limited to one per quarter.
